The Surgical Discharge Liaison Nurse will be located within ward 6A and is a member of the nursing leadership team. Reporting directly to the NUM, the role will utilize and share knowledge, skills and the essence of nursing to improve access, flow and discharge of surgical patients at Peter Mac. You will be patient focused and able to work across the surgical space to ensure that patients have a clear discharge or transfer of care plan. This will include complex care planning, liaising with the multidisciplinary team and ensuring that referrals are made to other facilities in a timely manner. You will be working as a team member whilst also being a self-starter, and able to manage multiple priorities.

Selection Criteria Essential: Do you have a minimum of 4 years experience in Surgical Nursing?
Essential: Do you have excellent Communication, time management and problem solving skills?
Desirable: Do you have an understanding of KPI's particularly associated with access and flow?
